Financial Assistance
Hunterdon Medical Center has a Financial Assistance Program for qualifying families with limited incomes and/or extenuating circumstances. To determine whether your family qualifies for this program, please call a Billing Representative and request an application. All requests for the assistance program will require the following:
- Completed application form
- Proper identification (driver’s license, valid passport, Social Security card, or birth certificate)
- Proof of New Jersey residency for the month of your requested service (utility bill, driver’s license with physical home address, copy of lease or deed, or a letter from an individual stating you live with him/her and how long.
- A copy of your last income tax form
- All applicable W2 forms
- Two most recent pay-stubs from each employed adult in the family
- A copy of the check-stub from the unemployment payment if either parent is collecting unemployment.
- Liquid Assets for each family member
- Medicaid eligibility – If you are under the age of 18, over the age of 65, blind, disabled or pregnant – you must show proof that you were screened for eligible Medicaid Programs by Social Services/Social Security.
